I acknowledge and agree with the terms, conditions and responsibilities listed below:

1. Sign a STANDARD RELEASE WAIVER FORM to be kept on file at SRA.
2. I must be at least 18 years of age.
3. I will be responsible for the use of the key that has been issued to me and for the actions of others that have access and use of this key.
4. I will abide by the Rules of the Rink and be responsible for adherence to the rules by those minors for which I have authority.
5. I will supervise those minors under 16 for which I have authority.
6. I am not required to accept the responsibility of minors who are present and unsupervised at the rink.
7. I have the authority to demand unsupervised minors leave the ice and to report their unmonitored behavior to the office.
8. I will relinquish my key to SRA on their command.
9. I understand this key is to be used only during posted open skating hours.  Use of this key at any other time is prohibited and may result in loss of rink privileges.
10. I understand that use of the rink is limited to members.  Use by others is prohibited without advance approval from the Ice Committee.

I will adhere to this agreement and the terms, conditions and responsibilities set forth within.
